This randomized controlled trial investigated the effects of a fasting-mimicking diet (FMD) and glycocalyx mimetic supplementation (Endocalyx™) on microvascular function in Surinamese South-Asian patients with type 2 diabetes (SA-T2DM). While FMD showed temporary improvements in BMI and HbA1c, it worsened microvascular health. In contrast, Endocalyx™ reduced urinary MCP-1 and improved microvascular health, an effect sustained at follow-up. These findings suggest that glycocalyx mimetics, but not FMD, may be beneficial for preserving microvascular health in this high-risk population.
