This study reports on a modified Delphi consensus process to develop a Core Outcome Set (COS) for nutritional intervention trials in older adults with malnutrition or at risk. Ninety-three and 72 participants completed two Delphi rounds, respectively, rating 38 outcomes. Eleven outcomes met initial inclusion criteria. A final consensus meeting with 15 participants selected ten outcomes for the COS: malnutrition status, dietary intake, appetite, body weight or BMI, muscle strength, muscle mass, functional performance, functional limitations, quality of life, and intervention acceptability. These ten outcomes will guide future research in this area.
