Nationwide Incidence and Clinical Outcomes of COVID-19 Vaccination-Related Myocarditis in Korea

Abstract
This nationwide study analyzed the incidence and clinical courses of COVID-19 vaccination-related myocarditis (VRM) in Korea. Among 44,276,704 vaccinated individuals, 480 VRM cases were confirmed (1.08 cases per 100,000 persons). VRM incidence was higher in men and with mRNA vaccines. Severe VRM (19.8% of total VRM cases) included ICU admissions, fulminant myocarditis, ECMO, and deaths. Eight sudden cardiac deaths (SCDs) were autopsy-confirmed as attributable to VRM, all in individuals under 45 years receiving mRNA vaccines. While VRM was rare, severe cases and SCD warrant close monitoring.
