This manuscript provides an accessible introduction to psychometrics, instrument properties, and scale development, using examples from the COnsensus-based Standards for the selection of health status Measurement INstruments (COSMIN) recommendations. It emphasizes the critical role of sound measurement in healthcare, explores latent variables and hypothetical constructs, and addresses issues of flawed measurement. A ten-step process for developing and validating new measurement instruments is outlined, demonstrated through a visuoperceptual measure for analyzing disordered swallowing. The manuscript serves as a resource for healthcare clinicians, educators, and researchers.
