This nationwide survey in Italy investigated the impact of the COVID-19 lockdown on the weight, lifestyle, and psychological well-being of 1232 patients with obesity (PWO). During lockdown, 48.8% gained weight, 27.1% lost weight, and 24.1% maintained their weight. Increased emotional difficulties (mostly fear and dissatisfaction), reduced physical activity, and changes in sleep quality were prevalent. Lack of online contact with obesity centers strongly correlated with weight gain. Multiple correspondence analysis identified two clusters: those with unchanged or improved lifestyles and those with worse lifestyles (unemployed individuals, experiencing depression, boredom, and increased food contemplation). Work status, emotional dysregulation, and lack of online communication with obesity centers were significant determinants of weight gain during lockdown.
