Abstract

This systematic review and meta-analysis compared stand-alone anterior lumbar interbody fusion (ALIF) with posterior fusion techniques (PLIF, TLIF, PLF) in patients with spondylolisthesis and degenerative disc disease. Twenty-one studies (3686 patients) showed ALIF resulted in significantly shorter surgical time and less blood loss than TLIF/PLIF, and shorter hospital stays than TLIF. Fusion rates were similar across approaches. Patient-reported outcome measures (PROMs) were mostly equivocal between ALIF and PLIF/TLIF, but favored ALIF over PLF for back pain (VAS, ODI, JOAS). Adverse events were similar across groups.
