This case report describes a rare presentation of serogroup B meningococcal disease (MD) in a previously healthy infant. The infant presented with meningitis, purulent acute otitis media (AOM), and paranasal sinusitis (PS). The case highlights the atypical manifestations of MD and emphasizes the importance of active surveillance for early detection, especially in regions with MD endemicity, like Tijuana, Mexico. The patient responded well to treatment and showed no sequelae.
