Abstract
Ketone bodies (KBs), alternative brain energy sources when glucose is limited, are investigated for their association with cognitive function. This study triangulates observational and Mendelian randomization (MR) studies to examine the relationship between 3-β-hydroxybutyrate (BOHBUT), a major KB, and cognitive performance. Observational analyses in the Whitehall II study (5506 participants) revealed a positive association between BOHBUT and general cognitive function. Two-sample MR using summary statistics from large consortia confirmed a protective effect of BOHBUT on cognitive performance and Alzheimer's disease (AD). Cis-MR analysis of OXCT1, a gene involved in KB catabolism, showed limited evidence of therapeutic modulation. The findings suggest a beneficial role for BOHBUT in cognitive health.
