Abstract
Cannabis can trigger acute cannabis-associated psychotic symptoms (CAPS) in people who use cannabis (PWUC). Using an international sample of PWUC (n=233,475), the study estimated the lifetime and last-year rates of CAPS requiring emergency medical treatment and examined correlates. Lifetime CAPS occurred in 0.47% (95% CI 0.42–0.52) and last-year CAPS in 0.19% (95% CI 0.17–0.21). Higher last-year risk was observed among younger individuals (RR 2.66 vs older PWUC) and those residing in Denmark (RR 3.01), those predominantly using high-potency resin (RR 2.11), mixing cannabis with tobacco (RR 2.15), and those with diagnoses of psychosis (RR 14.01), bipolar disorder (RR 4.30), anxiety (RR 2.92), and depression (RR 2.68). CAPS appear to occur in about 1 in 200 PWUC over a lifetime, with heightened sensitivity among young people and those with mental health vulnerabilities. Findings underscore the need for research and public education on risks amid cannabis legalization.
