This study analyzed the bioactive components of human milk (HM) and evaluated fortification choices to optimize nutrition for extremely premature infants. Donor human milk (DHM) showed lower lactoferrin and α-lactalbumin than mothers' own milk (MOM). Human milk-derived fortifier (HMDF) reinstated these components and enhanced antioxidant activity compared to cow's milk-derived fortifier (CMDF). Freshly expressed MOM fortified with HMDF is suggested as an optimal nutritional choice.
