Abstract
Preclinical evidence suggests antidepressants (ADs) may differentially affect mitochondrial energetics. This study investigated the relationship between mitochondrial function and bipolar disorder (BD) illness vulnerability, specifically treatment-emergent mania (TEM+). Participants with BD (TEM+ n=176, TEM- n=516) were classified based on whether their associated AD increased (Mito+; n=600) or decreased (Mito–; n=289) mitochondrial electron transport chain (ETC) activity. Generalized estimating equations, adjusted for sex, age, and BD type, revealed TEM+ was more frequent with Mito+ ADs (24.7%) than Mito– ADs (13.5%) (OR = 2.21; p = 0.000009). These findings suggest reclassifying ADs based on mitochondrial energetics, in addition to monoaminergic mechanisms, for improved risk assessment of TEM+.
